Form submissions not being caputured

For some reason, AC is not capturing all the field information on one of my embedded forms on my website. Any ideas?

Hi Wes. Welcome to the community. If it’s only one form, I’d suggest just recreating the form and, if it works, use that. HTHs. Cheers. Shane
