if the user has non html email or something blocking the “single tracking pixel” image, then opening it might not trigger an “open”
If they REPLY, does that get counted an open?

In the spirit of providing insight on this question, without the “1-pixel image” in your emails, replying does not “count” as an open and does not get tracked.
