Auto complete form fields for contacts already in my databse

I’m trying to set up auto form completion so when i send a landing page link to a list, anyone already in my database doesn’t have to enter their details again.
I was given this article that apparently explains how to do it, but when I get to my form URL, i can’t change it. And when I add the fields to my page URL, it doesn’t work:

e.g. if you follow the steps outline in the article where does it go wrong? What were you expecting? What actually happened?

“It doesn’t work” doesn’t really tell anyone enough to provide any assistance.
