Is there is a way to set up an approval flow so when a campaign is ready to go, a supervisor can be notified in order to review/approve before it is sent?

A great question, but I don’t think that there is an automated way to do this.
When you create a campaign, you can obviously send a test version of the email to up to 5 email addresses, as described in the following help article, but I am not aware of a way to do this as an automated workflow.

GilesMagee is correct, we don’t have a way to do this right now. I highly recommend submitting this as an idea at ideas.activecampaign.com, where our team check this frequently and seek to implement ideas that are well supported.
